Scheduling an Appointment for Lab Testing with Quest Diagnostics

You can schedule your appointment at a Quest Diagnostics Patient Service Center (lab draw locations) here: https://appointment.questdiagnostics.com/schedule-appointment/as-reason-for-visit.

• Please select "continue as a guest".  
• Make sure you select "all other tests" to proceed.
• In the option labeled Insurance, select "The person being tested doesn't have insurance." 
• Verify that the test center address you are interested in scheduling an appointment at is listed on our website by using the "Test Centers" button at the top of the page.  This ensures that you are going to a Test Center location that accepts our requisition form.
