The InSure® One™ test is an at-home test kit that you will need to pick up from any Quest Diagnostics testing center and complete the collection process in the convenience of your own home. Instructions will accompany your test kit, but it is important to make sure you complete the following steps prior to mailing your specimen to the lab.
- Go to the testing center and log in at the kiosk.
- You will be asked for your Requisition and ID. Do not give insurance information.
- The Phlebotomist will provide you with a screening kit and explain the process.
- Once you are home and ready to collect your specimen, complete the orange area of the InSure® ONE™ Requisition Form.
- Write “NA” next to the Patient Billing Information because you have already paid for the test.
- Read all instructions thoroughly on the back of the InSure® ONE™ Requisition Form prior to completing the steps. Watch this video for complete instructions – clinicalgenomics.com/quest.
- After following the instructions provided in the kit, make sure you include the following in the return envelope:
- A copy of your Grassroots Labs Requisition
- The Insure ONE Requisition Form
- Test card
